• Vision and Mission Statement of the Organization
• Name of Incumbent officers and their position and the age of the members
• Contact Person (if possible, the President of the Organization)
• Complete Address
• Telephone, Fax and Cellphone Number
• Accomplished Registration Form
• Constitution and By-laws
• Securities and Exchange Commission (SEC) Registration (if any)
• Letter of Recommendation for School/ Barangay/ Institution
• Note: The applicant youth or youth-serving organization must be in existence for at least 6 months at the time of its application.
