 I. INVITATION TO BID

The National Youth Commission, located at the 4 th Floor Bookman Building, #373 Quezon Avenue, Quezon City with telephone nos. 781-2386 local 113 /781-2323, is inviting interested bidders to bid on the following:

IMPROVEMENT/REHABILITATION OF THE SK ACADEMY BUILDING LOCATED AT PAGASA, QUEZON CITY

 WORK TO BE DONE:

 For New Construction:

• Construction of Laundry/drying area, dirty kitchen and additional 2 comfort room and 2 shower room each for both male and female Common CR, 2 units sink to be placed at the waiting area of the comfort room, including all materials to be used, plumbing work system, electrical system etc. (See attached proposed floor plan);

For Existing Building:

- To provide services for the installation of Nine (9) Portable/movable Air-conditioning Units, One (1) Split Type Air-conditioning unit including the materials to be used;

• Plumbing works/system, electrical maintenance including inspection of loaded area of the building, replacement of broken window glasses;
• Landscaping (includes plants and garden soil, plant boxes);
• Fabricate gate (based on the design) including the installation, concreting of path walk and repainting of existing fence (front).
• ELIGIBILITY REQUIREMENTS TO BID

 1. Name of company/business, address, telephone and fax number, year when the business was established including the number of manpower/employees;

2. Incorporation papers from the Securities and Exchange Commission (SEC) and /or from the bureau of Domestic Trade and Industry (DTI) for single proprietorship;

3. Value Added Tax (VAT)/ BIR Registration Certificate;

4. Track record in the last three (3) years (includes list of customer references and their addresses)

5. Certified photocopy of the latest business/license Permit for the current year;

• List of Bank References;
• List of Equipment owned;
• Certification under oath that each of the documents listed above is an original copy or a true and faithful reproduction of the original.
• APPROVED BUDGET FOR THE CONTRACT & SOURCE OF FUND: The approved budget is FOUR HUNDRED FIFTY THOUSAND PESOS (P 450,000.00) for this project chargeable against the NYC trust fund.
• DURATION OF THE CONTRACT: The project duration is Two (2) months, which will commence from the issuance of Notice to Proceed and approval of the contract.

• The National Youth Commission reserves the right to reject any or all bids without offering any reason, waive any required formality, and award the contract to any bidder whose proposal, as evaluated by the Committee, is the most advantageous to the government.
