Name of Organization Contact Person Contact Details (Mailing, E-mail, Cellphone or Landline numbers) List of Officers Ages of the Members By-laws of the Organization Vision/Mission Statements SEC Registration (if it's available) Certification from School or Community attesting to the existence of the organization No fees are required for applications. Please mail all documents to the YORP Secretariat at 4/F Bookman Building, #373 Quezon Avenue, Quezon City. |

Hi there, im new here and i want to register our group sa NYC. I've been browsing the website for hours now yet I can't seem to find where the registration form is. I've been to the local NYC ofc here pero naubusan n cla ng forms kya i was asked nlng to download it. i've also tried calling the ofc number ng NYC d2 s baguio but the fone doesn't seem to be working...help. :(
